All health professionals have a duty to protect your medical information. However in some instances some information will be shared, for example if you are referred to hospital or another service.

You can request access to a coded summary of your medical record online if you have registered for online services.
The Summary Care Record is a short summary of essential information about you which may enable medical staff to treat you more safely in an emergency.
It contains only basic information about you - your name, address, date of birth and NHS number. It also contains information about your medications and allergies. It does not contain information about consultations you have had.
If you are taken ill and need to attend a hospital or walk-in-centre, the Summary Care Record will help the doctors or nurses to treat you more safely.
You are automatically included in the scheme unless If you choose to opt out. To opt out you will need to complete a form and send it to the surgery. Please consider carefully the potential risks of opting out of the Summary Care Record before you do so.
National Diabetes Audit
Each year the surgery participates in the National Diabetes Audit. This allows us to assess our practice against guidelines and to identify potential gaps in local services. Taking part in this audit is a demonstration of our commitment to improve care for patients with diabetes. In order to participate we share some information with the National Diabetes Audit team. The type of informaiton is controlled by law and subject to strict rules of confidentiality.
